Also disturbing that a senior academic, who has presumably done much reviewing, ignores the core reviewer task of sitting back and saying OK what does this paper actually do, or potentially do with a bit more work.

Academic organizations have choke-points (e.g. approvals of new majors or classes) around which huge attention collects: files, meetings, layers of formal review. And after that you can pretty much do what you want! Nobody checks up.
