unfortunately. The way I think of it - they want to scrimp and save in all the places people can't fight back. Delaying payments to seniors and others is an easy way to hold onto money. It's exactly what the insurance companies do.

It’s a blend of things. It is a physician sometimes but many times the physicians determination in favor of the patient is overridden by the insurer themselves. Also capricious guidelines set by insurers that are followed by reviewed poison the well
