Maybe because he has a very strict dad, because of that strictness it seems that his dad view friends as obstacles
having not getting the attention from his dad lead Azuma to grow an obsession with success and perfection, and the wheel chair seems to symbolize a disability stem from that obsession

Chapter 58.3 implies that though Tokio and Azuma are friend, they never hang out at Azuma’s house (at the very least

while Tokio talk about Azuma a lot with his family, it appears that Azuma never get the chance, or perhaps, doesn’t want his family to know that he has a friend for some reasons

IDK maybe maybe Ishida plan to make Azuma the actual protagonist and make him an X and but the story is leading in Tokio's view

Just like JTTW where we mostly see Wukong on screen but the real protagonist is actually Tripitaka
