A community health needs assessment (CHNA) identifies a community's health issues to drive positive changes, but first, we need your help.

That's why the Cuyahoga County Board of Health and the Cleveland Department of Public Health are holding community prioritization meetings to identify Cleveland and Cuyahoga County's top health needs and issues.

West-Side session is on October 15 from 5:30-8:30 at Cleveland Public Library's South Branch on 3096 Scranton Rd. The East-Side session is on October 16 from 5:30-8:30 at Neighborhood Leadership Institute on 5246 Broadway Ave.

Participation allows you to support community health, and we will provide dinner and a $25 gift card to Dave's Market as a thank you for your participation.
