Questions remain about how the variation in approaches between Trusts highlighted in this report has impacted on patient safety, which currently remains unquantified and unanswered. Ultimately PSIRF must be judged on its implementation and effectiveness in reducing avoidable harm.

We welcome HSSIB undertaking this investigation, which makes clear that although PSIRF has now been rolled out across the NHS in England, its implementation is far from complete.
