For me I try to make myself uncomfortable if I wanna make something scary, really.

I like to look at my favourite media and see what it did to unnerve me, and then I wonder I can execute the same things that terrify me. For example, I always found balos from Cave Story (especially his final form when he gets damaged) to be unnerving, which helps with designing anything similar.
