I think it is okay for fiction to be a little blunt at times. Ursula Le Guin could be in your fucking face blunt at times, and its some of her most powerful work. Terry Pratchett is blunt as a sledgehammer.

I always interpreted it to be ambiguous. Sometimes it means boys(children) and sometimes it means boys(adult men), depending on context and tone. It could also be that the Westerosi patriarchy doesn't really differentiate between the two, especially not when used as a pejorative.
