self-hosting solves at least some of the problems, but not all of them (and it introduces some of its own).

Smaller scale is ALWAYS preferable. Avoids cruft :)

I've not seen that specific result. But it should be easy to mitigate by limiting which scans are being sent in the prompt

I work in medicine and there are genuinely good use cases. A tool that highlights sections of a radiology image that a radiologist might otherwise miss. Suggestions to changes to clinical notes (RBC is low, maybe add an anemia ICD-10 code). Lots of research uses too for prescreening pts re trials

Scott Hoffman (babydaddy) was a year ahead of me at Columbia. I had one of his classmates as a roommate in my first brooklyn apartment in about 2002 or 2003. Scott lived around the corner. I remember going over one day and seeing him work on the first album in his boxers. No idea it would blow up!

This is mostly due to 2 things: opening of subway line, and lack of construction laws. In the 1920's no housing was taller than 10 stories. Between 1900 and 1940 the population doubled (3.5m to 7.5m)
