Penetration is not impossible but, especially with high volumes infused, fluids piles up around your penis also, and that can create some difficulties. At lower volumes there’s usually no problems

Most people start with a 500ml. I personally prefer dex, but in case of diabetes dex is not recommended, so better saline.

6- does it hurt?: the needle insertion is just a little pinch, some people say dex solution stings a little bit at the beginning and saline doesn’t, I personally find it the opposite, but it’s fades away after few minutes. If done right does not hurt, the heaviness in your balls is a desired feeling
