The closest I've found has been GLMA (dot org). Managed to find an LGBT+ focused doctor's office. While I haven't delved into the more intense kinks with my GP, nor have I gone in locked, the few hints I've dropped were received more knowingly than expected so I doubt it would be a shock/problem.

My devious mind immediately goes to the tokens triggering different things like a piShock, cage vibrator, etc. and just stashing the tokens around an event like easter eggs for folks in the know to come find you and see what they get to make happen.

Ordered mine Oct 2021, managed to get through to ask for a status update a year later in Oct 2022, and finally received the cage another ~year later in Aug 2023. Similar to above, I had to tweet and tag them to get a response (despite DMs, email, and Whatsapp msgs). Initially quoted 6mos, took 2yrs.
