Best part about being an attending?

Responding to emails from residents and med students with a couple words, poor grammar, and an emoji.

Government shutdowns hurt patients.

❌ Telemedicine visits not covered → more ER visits
❌ SNAP/WIC paused → families can’t feed kids
❌ Medicare/Medicaid reimbursements stall → fewer providers, higher costs

Patient's cant get to nursing homes on gov't insurance!!!

Docs...
Stop telling little girls they’re “trouble” or asking if they have boyfriends. It’s gross, unsafe, and erodes trust in healthcare.

Talk to kids like this instead:
“What’s your favorite show?”
“Do you like to dance?”
“Tell me about your shirt.”

It's not hard!

I don’t know a ton about newborns but if their bill is well into the 20’s regardless if they’re 5 hours or 5 days old, that is not great

PJP pneumonia gets bactrim and in real life if they’re hypoxic I’ll probably do steroids, but according to Rosh you only do steroids if their PaO2 <70 or A-a gradient is >35

Spinal cord things are tough:
Central → Extension (elderly fall, arms > legs)
Anterior → Flexion (motor + pain/temp gone, proprio spared)
Brown-Séquard → Penetrating (ipsi motor/proprio, contra pain/temp)
Posterior → B12/Tabes (proprio loss, “drunk in the dark”)

Some named neck fractures just sound brutal:
Hangman’s? Bad.
Teardrop? Bad.
Clay shoveler’s? Doesn’t sound too bad. C6-c7 spinous process hurts, but probably not crazy

If you see a patient with a seizure and discharge from the ED - do you tell them they can’t drive for 6 months and put that in your note?

Only if confirmed on EEG? What if it’s from EtOH withdrawal?
