I don't think it's annually. The article says you renew your health care at the same time you renew your government ID or driver's license. For me that would be about 6 years after getting the card. The weird thing is there's no mention of how renewal would work.

This in a city of over a million people. I happened to buy an item last night and this morning the guy who delivered it was someone I'd met and talked to (in a different context) 5 days earlier.

I confess I don't have any conclusion to draw from this. It's just kind of weird.
