It’s actually very easy. In countries with decent health care systems these conditions are discovered, usually at puberty. In poorer countries they often aren’t, although Caster and the coaches most likely knew and knew Caster was actually a mediocre male athlete.

The racism argument is really stupid and anyone using it doesn’t understand how sport works. The biggest victims of Caster et al are black African female athletes who lost spots on local, regional and national teams, and local, regional, and national medals.
