This is compared to our neighbor corporate systems. I often hear about older docs who sold out to the corporation and then "things changed."
So many problems can be managed by a good primary care doc without a specialist referral. More cheaply, more personably, and with better continuity.

And it doesn't only exist in rural settings. It's generally impractical for us to be doing obstetrics, but one of my association's partners still admits his own patients. I do basic procedures including skin biopsies and OMT. We still try to be a one-stop-shop as much as possible. We are still here.

The comments in the piece about improving standards of care through academia are entirely true. But as we go through medical training, there is so much focus on the academic setting that many students are surprised that you can still practice to the full extent of #FamilyMedicine. I was.

My new patients tell me it's been hard to find a #PrimaryCare doc, and even harder to find one they like who:
-will put in the work
-gets to know them
-they can see on a timely basis for acute problems, and
-won't just refer them to an (expensive) subspecialist for everything.
