“For clinicians who have abandoned cricoid force… the qtn to be answered is why they employ any element of RSI at all. Rather than selectively abandoning cricoid force as a component, it should perhaps be the only component we retain”

Noradrenaline, metaraminol, phenylephrine or vasopressin?

This ovine cardiopulmonary bypass experiment suggests that the choice of vasopressors may affect renal oxygenation.

Heart-lung bypass lowered kidney blood flow by about half and lowered oxygen levels in the kidney tissue.

The brain's blood flow and oxygen levels were not much changed.

When noradrenaline was used, kidney oxygen levels dropped more than with vasopressin.

Does the choice of vasopressors affect renal oxygenation during cardiopulmonary bypass?

Using an ovine model, researchers found that intra-operative noradrenaline worsens renal medullary tissue oxygenation relative to vasopressin.

Can special breathing tests help doctors find out which children might have breathing problems during an operation?

Researchers were able to do the nitric oxide test in 91% of the children and the oscillation test in 61%.

Neither test helped doctors!

Can they predict peri-operative respiratory adverse events?

Post-anaesthetic care unit (PACU) and overnight intensive recovery units.

What is the impact?

It builds surgical capacity and system resilience.

Target-controlled infusions have been criticised as too slow for RSI, due primarily to the maximum infusion rate of most pumps (~1200 ml/h).

Effect-site concentrations at time-to-peak effect were nearly identical between target-controlled and manual infusions.

Cricoid pressure has been selectively vilified out of RSI in many settings, yet it has less risks, a better rationale & more evidence for efficacy than any other cited component of RSI

Why aren’t we doing it routinely for all adult tracheal intubations?

In a highly unusual turn of events anaesthetists are arguing about the role of cricoid. bsky.app/profile/anae... Blow me down with a feather!
"It is perverse that cricoid force has been abandoned selectively by many clinicians for not representing evidence-based practice when, compared with [other RSI elements], it is cost-free, better reasoned, has less potential for harm & more evidence of efficacy."
