Might be an odd pick, but the bad ending to Lego Island where you fail to catch the Brickster gave me nightmares as a kid.

I'm having fun on this island full of fun, wacky characters, I accidentally let this dude out of jail, he gets away, and the game rubs in my face that I let everyone down!
