We’re responsible for conducting our review and contributing to any multi-agency review that parish authorities convene. FDH doesn’t unilaterally control decisions made by city or parish leadership, but we do document what we saw, what was communicated, and how those choices affected operations.

Nobody at FDH can determine fault at this stage. Until a full review is conducted, we simply don’t have the complete picture. That review may show that:

an individual or team made an error,

procedures in place at the time were inadequate and need to be updated, or
