I draw my sonas surgery a bunch of different ways including with fantasy implants, but using the metal rod was just the simplest way to show a complex and varied surgery, especially given there's 2 and 3 piece inflatables with different placements and requirements.

No worries at all! He's a transexual man who's had sex reassignment surgery and opted for a vaginectomy a long side getting scrotoplasty, metoidioplasty, and phalloplasty, so he doesn't have both but it is an option some people can get!

In this piece he's drawn with ALT phallo (a graft taken from the thigh), placed above his metoidioplasty (tdick) and has a metal rod implant. Because of the placement he cant get a urethral lengthening so won't be pissing with it
