❓ Q: If my patient with pneumonia has a positive MRSA nasal PCR but respiratory cultures are negative for MRSA, do I need to continue vancomycin for a full course of therapy?

🩺 Correcting K and pH early is important for many reasons, with one being that it can help prevent progression to type 3 cardiorenal syndrome
🩺 Especially in critically ill patients, watch out for life-threatening renal complications (remember AEIOU) and initiate RRT early
🩺 Bicarbonate is often used a bridge while waiting for RRT and can be considered with severe metabolic acidemia, especially with AKI or hyperK
🩺 Persistent hypotension requires urgent evaluation and invasive monitoring - remember that NIBP readings may underestimate true BP in edematous and chronically hypertensive patients so a high index of suspicion is important
