This is meant to engage with the fact that smoking drastically increases your risk of negative health effects that come with smoking while on estrogen based HRT.

Real photos of the artist's penis and semen are also used as an artistic decision since she is transgender and proud to be. The artist's tongue is also present with estradiol and spironolactone tablets that seem to glow against the black and white tongue. This is once again related to the artist's experience as a trans woman.
