Actually already considered this with Fengo! Attached image/alt text explains it, but tl;dr if Astarion dies in the ritual and Karlach burns up Fengo decides that humanity is no longer worth it, and the only possible solution is to embrace Bhaal so he can force humanity to start again.
You'll be happy to know though in Fengo's evil ending he gets back together with Gortash after Astarion dies by being sacrificed in the Ascension ritual, with him and Gortash teaming back up and having evil yaoi while Fengo goes more and more insane and Gortash and him get more and more obsessive and twisted. Fengo slowly losing it more and more because he believes no one is worth saving after how many people who he loved and who deserved to live died in front of him, while Gortash gets more and more obsessed with helping him conquer the world to fulfill Bane's edict. They get sooo evil and twisted, Fengo probably CW cannibalism would cut chunks off Gortash and eat them because that way "a part of Gortash will stay in him no matter what". He would also probably keep Gortash in his room/the Bhaalist temple at all times, and if he has to go out Fengo will never let him out of his sight. Because he refuses to lose the last person who loves him. Fengo doesn't even really want to he just. Thinks Bhaal is his only way to punish the world for what it's done to him and the people he loves. Which I think is even more evil of me.
