Just because a bunch of us dig paediatric anaesthesia so much we built a website in our spare time. Really it's just another spot to hang out with other people who dig paediatric anaesthesia. www.justlittleadults.com #pedsanes #ansky

Ji et al. assessed whether the combination of dexmedetomidine and remifentanil, by reducing sevoflurane exposure, has any differential effect on neurodevelopmental outcomes in young children compared with sevoflurane alone. Read the new cover study: ow.ly/btn050WU27p

Important horse content.

Also when we sent out our thing to subscribers we included a link to this very important and inspiring story of a horse struggling its way into existence. Just more content we could all use today. vimeo.com/395927811

[New content klaxon sounds]. Yes. We put up new stuff about anaesthesia for CT scans in kids if that's your area of work. #ansky #pedsanes

There is some new content up at the site - a quick bit on providing anaesthesia for CT scans in kids. You can find it nestled around here. www.justlittleadults.com/imagingandir #ansky #pedsanes

I really dig this stuff we put up for those who look after kids all about central lines. Look just trust me it is niche but good. #pedsanes #ansky
justlittleadults.bsky.social
This week the site had a whole new range of good things on kids' central lines. Including practical stuff on line choice, positioning and the brachiocephalic approach. All at this spot right here. #ansky #pedsanes www.justlittleadults.com/central-veno...

But wait, there’s more (because value for your no money is what we’re about)… on the link right here you can find resources on types of lines, positioning lines and the approach to brachiocephalic insertion. #ansky #pedsanes www.justlittleadults.com/central-veno...

And for me (it’s AW on at the moment) when I had an elective CT scan list it was often surprising how even seemingly straightforward induction with easy FMV was associated with some gas in the stomach (which also —> nausea etc surely).
justlittleadults.bsky.social
In the more straightforward scenario it is pretty common practice either after initial care or if you have had to go back to bagging to use an NG to decompress as an interim step to aid re-oxygenation.
justlittleadults.bsky.social
Sounds like things got there?

In the meantime we noticed we had not mentioned the role of NG etc in these kids and will be updating the post to reflect that point.

Has some upsides. Can be an issue with seal if in for preoxygenation etc.
justlittleadults.bsky.social
And if you’re up for it this sounds like something we could maybe use to initiate a case chat style post (de-identified appropriately etc.) in the hope it generates useful chat.

Either message here or the bottom of each page on the site has an email link :)
justlittleadults.bsky.social
It’d be great to hear how things went for you (feel free to message or contact another way if you’d rather not here).
justlittleadults.bsky.social
Initiating treatment for the underlying cause for oedema might also help. Somewhat assuming other things like NIV were already being tried.
justlittleadults.bsky.social
Early use of second gen SGA in the hope of better oxygenation plus gastric port/seal might be considered if patient obtunded prior to paralysis….
justlittleadults.bsky.social
Broadly the tips for good face mask technique and a bit of head up seem reasonable. But this sounds pretty extreme.

But if paralysis working:
- Prompt intubation (noting there is a whole separate chat to be had about developing strategy in advance of the attempt to optimise oxygenation etc.)
