Interesting question, i.e. I have never thought about it. I brainstorm titles, but I never think about covers. They are usually from a major scene in my outline. I sometimes agonize over illustrations, either too many good scenes or not enough.

They are important scenes, so generally not. I had 1 cover where I wish I'd done something else. It was built around what turned out to be a Maguffin. The mtn. in the third cover below is mentioned, but nothing much is done with it. I'm not a panser. I have a general plot laid out in advance.

beta are very mature text. Illustrations are commissioned long before betas take place.

I write serials, so covers are quite early in the process. I may have 1/4 of a volume written when I order the cover. So I need a clear idea of the general course and high point of each vol.
