Of those techniques it is pretty much the only one that carries the risk of making airway Mx harder, in clinical situations where it is already potentially challenging.

Hence it being the first thing to go, for me at least!

Definitely would not teach cricoid as *the* essential preventer of aspiration in isolation.

It is however part of the suite of techniques we use (including high dose relaxant, available suction, NG aspiration etc) in these patients to reduce aspiration risk.

I’ll use cricoid pressure for any RSI-type situation as the default but if there’s any concern that it is impairing airway management it comes off straight away, and brief team as such. If you set the cricoid removal threshold low the mental load is minimal imo.

Also having experienced both types of anaesthetic I DEFINITELY preferred the TIVA experience…

I use gas for spont breathing via SGA, getting that to work with TIVA is something I find very hit and miss (although the trainees seem to love it). Also use bolus induction plus gas for sickies.

Everything else - if it needs and ETT I use TIVA now, always remi added in as a ‘smoothing agent’ 😆

I’m always ultrasound guided these days, it’s immediately available, and first pass success is pretty much guaranteed. They even switch on instantly these days!

I’m pretty sure I’m at least as fast (maybe quicker) than when they were all done blind.

It's World Cancer Day and 10 years ago this year I was diagnosed with breast cancer. I can't believe it was a decade ago

This is me in hospital waiting to have a mastectomy. I'm smiling but god I was so anxious, so worried I wouldn't wake up from the anaesthetic

Went onto the Facebook "Medical Consultants" group: it's clear that NHS consultants are spending far more time than we should be worrying about avoiding (or reducing) 5-figure pension tax bills.

Sadly, the usual and safest answer is "cut your hours"
