Why is it not Sojiro she's looking at? Because in the universe of P5X, Cafe Leblanc works differently and acts as a separate space from the original P5's version of Leblanc.

And Ren is the only one who has ever worked behind the counter of this special version of the cafe.

In P5X , not only is Makoto the only one of the original PT members to have an animated wallpaper of her out of her Metaverse costume, but this unique wallpaper also features her hanging out at Leblanc... and presumably smiling at Ren who is working behind the counter.
