He doesn't want anyone else knowing about his diapers so he lies about seeing a doctor about it, hoping that it will just pass. Then he finds out that he might need diapers while also needing diapers. Berny offers Jack a change, but did he mean it or is he just joking? Regardless, the idea makes Jack feel funny.
