Undocumented immigrants, under longstanding policy, have been and continue to be ineligible for federally funded health coverage. Medicaid, ACA, and Medicare eligibility cuts in H.R. 1 apply to lawfully present immigrants.

Democrats are pushing to reverse Medicaid and ACA cuts. Contrary to Republican talking points, that would not in any way expand health care eligibility for undocumented immigrants.

States may need to establish new mechanisms to purchase & administer vaccines outside VFC, which might require a SPA + approval from CMS. And while states could use state-only $ to pay for services not covered w federal #Medicaid $ this is unlikely given severe budget constraints facing many states.

NC's Healthy Opportunity Pilot was a promising demo: An analysis by the UNC School of Medicine found the program saved $1,000 annually for each of the 13,000 #Medicaid recipients enrolled in the pilot, which covers three rural swaths of the state. www.nytimes.com/2025/09/29/h... #healthpolicy

Good thread if you need a refresher on how #Medicaid could be be impacted by a federal government shutdown 🧵

Thread on the looming government shutdown and how it will affect #Medicaid and CHIP. Federal $ for states will be unaffected. People enrolled in Medicaid and CHIP can continue to access benefits and health care providers will continue to be reimbursed for the services they furnish to enrollees (1/x)

The 2024 CPS data reflected steep Medicaid coverage loss due to unwinding of a COVID-related Medicaid protection, partly offset by ACA marketplace gains due to enhanced premium tax credits and other policies. Those credits will expire soon if not extended. tinyurl.com/yc3rt45t

As always, today's CPS data release shows that states that expanded Medicaid under the ACA had MUCH lower uninsured rates than the 10 holdout states that still haven’t expanded. tinyurl.com/bdfadufc

“The changes mean that about 10 million fewer people are expected to have health insurance over the next decade,” per CBO.

Important story from Yasmeen Abutaleb and @maevereston.bsky.social on the costs and consequences of Trump’s budget bill — and, especially, the big decisions looming for states.

Glad to have supported this team!

One highlight: Even though the OBBBA offers states modest grants to update data systems (totaling $200M nationally), this amount compares unfavorably with the roughly $50M Georgia spent on its work requirement administration program. #Medicaid #WorkRequirement

North Carolina also announced 3% cuts to Medicaid provider reimbursement rates last month. ncnewsline.com/2025/08/13/n...

I anticipate we'll see more states acting similarly, especially once 2026 legislative sessions begin & we have a better understanding of anticipated budget shortfalls.

Nationally, #Medicaid reimbursements account for 42% of FQHC revenue, compared to just 1/10 from congressional funding. That balance means any pullback in Medicaid can push clinics into crisis. W/out additional funding...advocates say many FQHCs may be forced to shut down or reduce services.
