Case report describes a 61 y/o woman with IgG4-RD presenting with retroperitoneal fibrosis & autoimmune pancreatitis, followed by sequential development of papillary thyroid carcinoma and diffuse large B cell lymphoma

ACR Open Rheumatology
doi.org/10.1002/acr2...

#Medsky #oncology
Pathologic and imaging features in a patient with IgG4-related disease (IgG4-RD) and subsequent malignancies. (A and B) Retroperitoneal biopsy showing hallmark features of IgG4-RD. (A) H&E staining reveals storiform fibrosis with dense lymphoplasmacytic infiltration. (B) Immunohistochemistry highlights abundant IgG4-positive plasma cells. (C and D) Histopathologic features of papillary thyroid carcinoma. (C) H&E staining shows typical papillary architecture with fibrovascular cores. (D) CK19 immunostaining demonstrates strong and diffuse membranous positivity in tumor cells. (E and F) PET-CT findings during disease progression. (E) Whole-body PET scan reveals metabolically active lymphadenopathy across multiple regions. (F) Coronal PET-CT fusion image shows intense FDG uptake in retroperitoneal and pelvic masses. (G and H) Lymph node biopsy confirming diffuse large B cell lymphoma. (G) H&E staining shows diffuse infiltration of large atypical lymphoid cells with prominent nucleoli. (H) Immunohistochemistry for CD20 shows strong membranous positivity, confirming B cell lineage

Study identified a panel of serum soluble mediators as potential biomarkers for monitoring proliferative lupus nephritis. Early reductions in syndecan-1 and VCAM-1 were predictive of 12-month treatment response in proliferative LN

Arthritis Care & Research
doi.org/10.1002/acr....

#Medsky
Serum soluble mediator profiles distinguish proliferative lupus nephritis (LN). Soluble mediator concentrations were measured at the time of kidney biopsy. Bootstrapped LASSO regression was used to select mediators associated with proliferative LN. (A) Forest plot of associations between these LASSO-selected soluble mediator concentrations and proliferative LN, as determined by univariate or multivariate logistic regression incorporating all selected mediators. (B) Forest plot displaying the area under the curve (AUC) for the performance of LASSO-selected soluble mediator concentrations in predicting proliferative LN. (C) Receiver operating characteristic curves for multivariate logistic regression models predicting proliferative LN: LASSO-selected soluble mediators (Model 1), clinical/demographic factors (race, ethnicity, biopsy history, SLEDAI, C3, C4; Model 2), and combined LASSO-selected soluble mediators and clinical/demographic factors (Model 3).

Longitudinal study in Japanese women found increased serum urate levels several years before menopause, with the largest increase occurring during peri-menopause, particularly in overweight or obese women and those with lower eGFR

Arthritis & Rheumatology
doi.org/10.1002/art....

#Medsky
Longitudinal trends in serum urate and hyperuricemia. Legend: Mean serum urate levels (A) and the proportion of hyperuricemia (B) with a 95% CI were described. The date of menopause was defined as the median date between the two consecutive visits immediately before and after the self-reported menopause occurrence. The time from menopause was calculated for each visit. Participant data were then summarized by years from menopause. eGFR, estimated glomerular filtration rate; CI, confidence intervals
