Virtually endless combinations of expressions exist in human bodies. Only 1 in 5000 is visibly intersex.
Sex exists on a spectrum. Various combinations of chromosomes, hormones, external genitalia and internal reproductive structures exist. Sometimes these characteristics are visible at birth, sometimes they appear during puberty, but often they are never physically noticeable.
