This study of 1,207 older adults receiving home-based primary care, seeing the same GP for ≥75% of visits was associated with a 31% lower hazard of home ambulance services, a 39% lower hazard of emergency department visits, & 35% lower hazard of hospital admissions

In 32 #PrimaryCare clinics, higher-intensity strategies cut opioid doses & improved pain/function screening. Clinic- and prescriber-level support lowered overall costs vs. system-only approaches.
