It would normally follow two elevated PSA tests. 10 years ago they would do a biopsy. Now they would do Multiparametric MRI first, which can be set to ignore very low-risk cancer foci.

PIRADS 4 indicates biopsy. 3 would now be surveillance even if G6 suspected.
