Bias is imposed from on high. I know a few hard-working journeys on popular newspapers with questionable ethics, who feel powerless to challenge the crazier aspects of what gets published.

How would you fix the terrible media thing? Been mulling this, with hypotheses like "Media owners in the UK must be British citizens and resident in the UK" and "all media organisations must be at least 50% owned by their employees". I wonder if this could make a difference.
