Did the multiple, uncoordinated efforts to create such transfers trip over each other's feet and create the very problem to be addressed?

Um, no? Modern buses don't have a rigid axle connecting their rear wheels. There may be a sloped bump between the rear wheels, but there isn't a step. And their rearmost door is behind the rear wheels anyway.

Well then.
It seems like the problem is not "no light rail can possibly do" but that a feature inappropriately borrowed from *intercity* *heavy* rail is a bad fit for urban transit.
