That's only really true in utopian stories. Take D&D for example. Most people will not see the amount of gold that adventurers get on the daily. In 3e a skilled laborer could make 9 gold a week. A minor healing potion costs 50 gold. Even in fantasy and sci-fi health care is for the rich and powerful
