When I do it myself I plug my nose using my thumbs and then use my index fingers to push my ear lids closed. (I learned today that the ear lid is properly called the "tragus.") The cup with the straw has to be sitting on a stable surface. Then you just kind of do it all at once and it will work.

I've had a lot of luck with this procedure:

1. Cup
2. Straw
3. Water
4. Plug your ears and pinch your nose. Get help here if necessary/you are a child
5. Drink the water through the straw while #4 is happening
6. Stop drinking the water
7. Release ears/nose

Your hiccups will stop if you landed it
