I think one reason for the reliance on private insurers is the federal system--gives many more opportunities for establishing dominancein a niche, giving a political base to resist reforms and a national solution.

IIRC some airmen started giving out candy. Can't remember if they dropped the candy on the approach to the airport or handed out in person.
