IMO, The third sentence seems to cut against your interpretation: “[w]hen a creature moves into or within the area, it takes 2d4 Piercing damage for every 5 feet it travels.” The “area” is the 20 foot radius sphere. The modifier “ground” locate where the spikes are growing from within that “area.”
