You can get a cable that connects to the back but I wear without and it's fine. So I have nothing back there, the metal is strong enough to just have the cage coming down

I went to a store that had a sizing station and had one of the guys working at the store help me. Sizing is incredibly important! You can do online, just read everything and be honest with what they instruct. I can wear it indefinitely if I had to.

It's really not bad at all. The only times I get uncomfortable is when doing sit ups/crunches or the leg press at the gym since it drives your lower back into the seat. Otherwise, outside the gym its all good
