Everything in the NHS should be based on two prime considerations:

1) What is best for the patient
2) What is best for the NHS overall

All pathways and policies should be built around these, and the funding should then fit the pathway.

Anything else is just counter-productive.

One of the more difficult things about being a doctor in training is that, almost by default, you are *not good enough*. You’re always expected to be improving, always having to prove you’re good enough for the next job, the next step up, the next stage. You never get to just *be* at your level (1/)

Didn’t get shortlisted for interview for dualling ICM HST. On the plus side, I get to rejoin my first hospital for my ST4 job, and can now go LTFT. But it shows the increasing bottlenecks for specialty training; the cutoff has increased by over 50% from usual (18 vs 12) …
