Software engineers and architects are not personally liable for the consequences of their mistakes. Defects are impossible to eradicate, but few bridges collapse from faulty design because a bad sign off by a licensed civil engineer ends a career. Nothing focuses the mind like a hanging.

A long term solution to software reliability is licensed software engineers and architects. When boiler explosions killed people in the industrial revolution, certified engineers began to be required sign off on boiler designs and be held liable for failures. The same should happen to software.
