There are two kinds of people: those on the side of right, Nazis, and bigots who call out false dichotomies.

The way to win an argument always to assuming bad faith in those with different opinions—label them as racist, bigots, or Nazis, and eventually, they'll come to your point of view.

If you're a young boy with gynecomastia, you should go get that treated—that's gender-affirming care for a cisgendered boy, and no one bats an eyelid.
However, if it's a trans boy getting top surgery, suddenly people are more concerned. That's pure, hateful transphobia, and that's why.
