For #Epeeps who are doing all PFA for afib, have you had patients with refractory afib after multiple RF ablations who you’ve had meaningful success after PFA? If so what was your approach?

Tough transseptal due to septal aneurysm, PFO, and thick limbus with a narrow window. Had to shepherd’s hook it in the LA to position the catheter in the RIPV. #epeeps

Congratulations to Lena Rivard on the @ahascience.bsky.social presentation of the BRAIN-AF study results.

Major observation is that cognitive decline occurs in many « low risk » AF patients.

Unfortunately, Rivaroxaban 15 mg daily did not impact this risk in this double-dummy RCT.

Similar low rates of stroke post ablation (no difference with laao vs OAC) with no significant 23% lower rate of major bleeding.

Here's one from the archives. 81 y/o retired physician with palpitations who documented this ECG (NOT pulse ox) on an @Apple Watch. EMS shock x1 for VT & was found to have 3v disease on LHC, s/p CABG. Pt doing well >1 month post-op.

PVC-induced cardiomyopathy. Targeted distal to the left posterior fascicle. Zero PVCs post ablation. Left posterior fascicular block noted at the end of the procedure but resolved within 15 minutes. #epeeps
