5/ Beyond the sleep example, there are other experiences people have the might differ. Most people taking GLP-1s want to reduce their appetite - how should they rate the appetite item? Most importantly, we probably want to know HOW people are filling out this questionnaire, and right now we don't...

4/ Why does this matter? Well, I have ADHD and thus have "Trouble concentrating" nearly every day, but it doesn't bother me so I rate that as "not at all". But in the few periods when I've been so stressed or ruminative that I couldn't concentrate I would have rated it a 3 because it bothered me.

3/ Because they had actually filled out the PHQ prior to these questions, we asked them how they had filled it out: based on the frequency of the Sx or based on the frequency with which they were bothered by the Sx (or a mix of both). Again, high variability, and few people focusing on 'bothered'

2/ We gave people the hypothetical: "Imagine you overslept every day for 2 weeks but it didn't bother you" and asked if they would select 0 "Not at All" or 3 "Nearly Every Day". Unfortunately, people with the same experience (oversleeping) said they would respond very differently to the PHQ:
