I do both, but I find the decks that end up grabbing me are the ones that start as an idea.
I find that if I start with a commander I end up with the same deck as everyone else, which is not my thing, but if I start with an idea I can build towards something unique.

I have a Glarb that manipulates the top of the deck for effects like this, and Lammastide Weave. They're always a blast
