Overview of the recommended management for systemic lupus erythematosus (SLE). This schematic illustrates the approach to SLE treatment, with organ-specific therapies presented in separate boxes and stratified by disease severity. The order of the medications in each box does not indicate preference unless noted otherwise. Lupus nephritis treatment is addressed in the 2025 American College of Rheumatology Guideline for the Screening, Treatment, and Management of Lupus Nephritis. ABBREVIATIONS: ACLE, acute cutaneous lupus erythematosus; SCLE, subacute cutaneous lupus erythematosus; CCLE, chronic cutaneous lupus erythematosus; CNI, calcineurin inhibitor (cyclosporin or tacrolimus); HCQ, hydroxychloroquine; CQ, chloroquine; CCB, calcium channel blockers; IL-1 blockade, Interleukin-1 blockade; IS, immunosuppressive therapy; IV CYC, intravenous cyclophosphamide; IVIG, intravenous immunoglobulins; LCV, leukocytoclastic vasculitis; MPAA, mycophenolic acid analogs; NSAIDs, non-steroidal anti-inflammatory drugs; PDE5i, phosphodiesterase type 5 inhibitors; PLEX, plasma exchange.
