Medicare is well-run, overall. The problem isn't in how it is run, the issue is the fundamental legal and regulatory framework that, to Mark's point, is not built to support folks of limited resources when they get sick or need chronic disease treatment.

That doesn't even include the indirect ramifications. As a result of all of this, Harvard shut down their Center for Primary Care, which is/was their primary research and education hub for primary care faculty and students interested in pursuing primary care professions.

I have this discussion with my patients when we consider starting psychiatric medications. Different meds have different withdrawal symptoms and severity, and that can help guide patients' decision-making. Definitely a recognized phenomenon.

As a Jewish person working at one of the targeted institutions, I continue pointing out that so many of the people losing their jobs are, in fact, Jews (including myself). They are hurting us.

It is terrible indeed. But the large systems here (MGB, BILH, BU, Tufts, Atrius) are overwhelmed by demand and despite trying to recruit more doctors, there are very few people applying for the positions. The problem is so complex, and the willingness and resources to address it just aren't there.

As a PCP in Boston, this story does resonate. But it's far too late, so many of my colleagues and I have been so brutally beaten down that our whole generation is functionally lost. We want to help patients, but it's just not worth the pain of doing so.
