If the alternative is increasing the norepi dose, it may be worth noting that both the VASST and VANISH trials had a slightly higher rate of life-threatening arrhythmias in the norepi group compared to the vaso group (secondary outcome, non-statistically significant). Neither had data on QTc AFAIK.

I’d love you take on steroids for secondary bacterial pneumonia complicating influenza. If there’s a proven or very likely bacterial infection with high CRP I lean towards steroids despite the lack of evidence. What are your thoughts?

IDSA say 12 months. IMO, it depends on how sick the patient is. If the cultures are more than 6 months old (older IDSA cutoff), and especially if there are more recent ones that grew more susceptible bacteria, I might wait with the broader spectrum coverage.
