By standardizing volume and minimizing transport delay, TTP becomes a powerful predictive tool for stewardship.

🎓 Great work from Mélanie Bouillon's thesis.

You can't interpret TTP without controlling the basics:

📉 Volume: We found a "threshold effect" at 3 mL. Below this, detection crashes. 🚚 Transport: Delays shorten machine TTP (bacteria grow in the tube) but delay the final result.

Volume remains the critical factor for sensitivity.

🦠 Is that Coagulase-Negative Staph (CoNS) a pathogen or a contaminant?

• True infections grow faster (Median ~15.5h) • Contaminants grow slower (Median ~21.2h)

💡 Our data suggests a cut-off of 25h (rather than the classic 20h) better captures true infections while flagging contaminants.

Time matters! 🕒 Our data shows TTP varies significantly by species

1️⃣ TTP < 12h: Strongly suggests S. aureus (Sp 0.95) or Enterobacterales (Sp 0.93). 2️⃣ TTP > 15h: Effectively excludes dangerous Streptococci (S. pneumoniae, pyogenes, agalactiae).

TTP is a free biomarker available 24/7. Use it!

🧫 In our lab, we use:
✅ Disk diffusion (classic but reliable!)
✅ MIC by Sensititre (for precision)
✅ E-test strips (when we need flexibility)
✅ Vitek 2 (for speed and automation)
What about you? Any tips or tricks to share? 😊
