pediatric intensivist #PedsICU CHU SteJustine, Univ de Montreal
Father and grand-father! Interested in pediatric ventilation, PARDS, neuromonitoring,...and environmental protection.
Absent/low drive is associated with higher mortality and later ICU discharge.
Excessive effort (very hig Pocc, 25-30 or more) also associated with adverse outcomes especially in patients with PF<150

Absent drive is very frequent (80%+ of the time)

Interesting cohort (adult) study in Canada on the association between respiratory drive and outcome. Limited by the observational design, but well done… and supporting the Lung and Diaphragm protective ventilation concept.

Mobilisation massive !

Interesting study by #LARed network, describing the respiratory support in 7374 #PedsICU patients.
NRS used in 84% patients, with a 85% success rate 👍
NIV failure not associated with worse PICU LOS or vent duration compared to primary IMV

Great Editorial note in @pedcritcaremed.bsky.social by @robertctasker.bsky.social on the advances in knowledge about #PARDS since PALICC-2, and some great suggestions on what still needs to be done.

Great editorial by N Yehya and @markpicu.bsky.social about the importance to conduct research in #PedsICU and the critical need to use alternative model to obtain the parental consent, in those critical situations.
Based on great data by Furyk and coll.

Agree ! It's time for Canada to get closer to friendly countries that still share our moral vision!

Interesting paper by @benrwhite.bsky.social and #Bacon large cohort : 42% of infants intubated with bronchiolitis have #PARDS, and this is associated with adverse outcomes.

Of the 10 most widely read articles in JAMA Peds in 2024, 4 relate to screen time and are worrying...
It is important to follow the recommendations of pediatric societies!

By the way, vaccination is safe...

Great collaborative work by A Kawagushi /P Jouvet on Prolonged vent in #PedsICU across the globe.
A complex and frequent (25% prevalence) condition, with high mortality rate.
Heterogeneity reflecting between site differences...and the lack of evidence...

Important RCT conducted in 1800+ adults with different type of respiratory distress, testing NIV vs HFNC.
Interestingly, median NIV use duration was 10-12hrs...

This seems very different from #PedsICU NIV practice (infants/child are more compliant!)...extrapolation should be prudent!

2024 Lancet Countown...
We are falling behind schedule, + more impact on the planet, and these delays will be very costly (in money and human lifes). Healthcare systems (esp in rich countries) contribute a lot, and have rising GHG emissions (+36% since 2016).
