The highest rating is Adult Only, which is the only rating banned on consoles for example. (And Epic store apparently)
Platforms (online stores or consoles manufacturers) can discuss and challenge IARC rating with publisher if they think it is not the right one, using very specific game sequences.

No it is not. It is the publisher/developer responsibility.
For digital only games, this is a purely declarative process. If it is proven that you lied in your declaration to get a lower rating, platforms can remove the game from stores.

I would say "realistic graphics" instead of just graphics for the meme.
Visuals are important in games, and a good stylized or minimalistic art direction can do marvels for your game feel.
